That's a hole made by either a bark beetle or woodpecker
Proably so. I don't know if it's true of other woodpeckers, but pileated woodpeckers excavate oblong holes when feeding and perfectly round ones for nesting. I found this an interesting read: http://www.ct.gov/dep/cwp/view.asp?A=2723&Q=416874
