Maybe look into subgenus Scaphoideus http://bugguide.net/node/view/881085 Looks the most like Scaphoideus incisus, but there aren't that many photos in the guide of the nymphs of the other species in the genus, so it could be another one
I would not exclude Cicadellidae, but I am really not sure to which family it belongs.
