A completely white mushroom. Its cap is about 3 to 4 inches in diameter and it has a small veil below the cap.

Habitat:

Deep forest near a stream.

I have little experience with mushrooms, but it sort of looks like a destroying angel, maybe Amanita bisporigera. When you found it, was it emerging out of a white cup-like structure, a volva? Take a look at this: http://en.wikipedia.org/wiki/Amanita_bisporigera
