Field Notes

Description:

Four inch wide cap. Creamy off white color.

Habitat:

Found 20 feet away from a small stream near a large rotted tree. Surrounded by leaf litter. No other similar fruiting bodies found close by.

The first shot is amazing! It's hard to tell based off just pictures, but it somewhat resembles an Agaricus silvicola, the Wood Mushroom; http://www.identifiezleschampignonsen3clics.com/images/stories/articles/champignons_de_nos_regions/comestibles/agaricus_silvicola_0221.jpg The things that are making me uncertain about that suggestion is that there is no yellow bruising on the cap or stem, that the cap is very conical and this does not occur in any of the stages of developement for A. silvicola. But as for everything else, to my novice knowledge, it seems to be a Wood Mushroom.
